BIRTH DOCUMENTS PROCESSED AND ISSUED   Figure 1 shows the number of birth certificates processed and issued by CRS Lipa City Outlet during the 1st quarter of 2022. A total of 61, 316 or a decrease of 7.7 percent compared to the same quarter of previous year at 66, 462, were processed and issued.   January recorded the lowest birth request at 16, 420 while March got a total of 26,612.   Figure 1. Total Number of Birth Documents Processed: 1st Quarter 2021 and 2022     DEATH DOCUMENTS PROCESSED AND ISSUED   During the 1st quarter of 2022, a total of 3,930 requests for death certificates were processed by CRS Lipa City Outlet, a decrease of 1,408 or 26.37 percent compared to the same period of 2021.  March posted the highest number of death certificates processed and issued for both years.   Figure 2. Total Number of Marriage Documents Processed: 1st Quarter 2021 and 2022   PSA CRS Lipa City Outlet issued a total of 11,519 marriage documents during the 1st quarter of 2022; or an increase of 21 marriage documents or .18 percent from the previous year of the same period.    CENOMAR PROCESSED AND ISSUED   Figure 4. Total Number of CENOMAR Processed: 1st Quarter 2021 and 2022   Figure 4 shows that during the 1st quarter of 2022, the PSA CRS Lipa City Outlet processed a total of 15,396 requests of CENOMAR. A decrease of 1.37 percent for the same period of last year of 15,610 was noted.   The month of March posted the highest number of requests at 6,138.   ELECTRONIC ENDORSMENT   PSA CRS Lipa City Outlet received a total of 2,194 Civil registry documents requested for electronic endorsement. Seventy-nine (79) percent or 1,735 of these requests were approved by the Central Office while 459 were disapproved. Reasons of disapproval are the following: 1.    Different specimen signature of certifying officer 2.    Submitted documents not certified 3.    With prior endorsement 4.    With existing record in the archive 5.    Wrong form used 6.    Blurred/unreadable entries       DECENTRALIZED COPY ANNOTATION PROCESS (DeCAP)     A total of 1,999 annotated civil registry documents (CRDs) requested for loading through the Decentralized Copy Annotation Process were received by the Lipa City Outlet during the 1st Quarter of 2022. Out of these numbers 648 or 32.42% were approved and loaded in the database, 200 or 10% of annotated CRDs were disapproved due to problems and the rest are in process. The following are the reasons of the disapproval of request for annotation of CRDs:   1)    Lacking or wrong supporting documents 2)    Supporting documents not certified 3)    Wrong format of affidavit 4)    Double/multiple registration 5)    Forwarded to CRS due to error in the Electronic Annotation System   Also, during the quarter the outlet processed and approved 1,140 annotated CRDs received during the previous quarters.      RA 9048 still the highest number among total received civil registry documents during the 1st quarter of 2022   Petitions for correction under Republic Act 9048 recorded the highest share among total received CRDs with a total of 1,650 petitions. It was followed by petitions for Supplemental Report, 217; and Republic Act 10172 with a share of 116. The outlet also processed combo annotations. Eleven CRDs with combo annotation of RA 9048 and 10172 and 5 CRDs with combo annotation of RA and supplemental were processed during the 1st quarter.   FREE ISSUANCE OF CIVIL REGISTRY DOCUMENTS   During the celebration of Civil Registration Month and Women’s Month, the outlet issued fifty CRDs to clients in the special lane and women clients in the regular lane who have no record in the PSA database.
