DeCAP or Decentralized Copy Annotation Process is an online processing of annotation of civil registry documents (CRDs) at the PSA Regional CRS Outlets affected by RA 9048, RA 10172, and Supplemental Report (1-2 missing entries and approved supplemental report by NSCRG).     Number of received and processed annotated civil registry documents (CRDs) in 2021 increased by 358 In 2021, a total of 8,766 civil registry documents were received in PSA CRS Lipa City Oulet located at the Regional Statistical Services Office IV-A. It is higher by 358 civil registry documents than received documents in 2020 at 8,408. Received annotated civil registry documents (CRDs) was highest in January (1,167) and lowest in May (353). On the average, about 731 civil registry documents were received monthly. This is equivalent to about 24 civil registry documents per day.   Figure 1. Number of processed annotated CRDs by month   Quezon shared the most number of received and processed annotated civil registry documents Among the provinces in CALABARZON, Quezon had the highest number of submitted civil registry documents with the total of 2,620. It was followed by Batangas, Laguna and Cavite with 2,429, 1,982 and 1,573 respectively. Rizal had the lowest submitted civil registry documents. Moreover, PSA CRS Lipa CIty Outlet received a total of 27 civil registry documents from other provinces outside CALABARZON.   Figure 2. Number of processed annotated CRDs by province   RA 9048 had the highest number among total received civil registry documents in CALABARZON in 2021 Petitions for correction under Republic Act 9048 recorded the highest share among total received CRDs with a total of 7,614 petitions. It was followed by petitions for Supplemental Report, 587; and Republic Act 10172 with share of 565.   Figure 3. Number of annotated CRDs by type of petition   Number of approved civil registry documents in CALABARZON increased In 2021, more civil registry documents were approved compared to 2020. Figure 4 shows that in 2020 only 45.22 percent or 3,802 out of 8,408 received CRDs were approved at RSSO due to voluminous backlogs and impact of pandemic; thus, some of the received civil registry documents were transmitted to Central Office for rescue process. While in 2021, 77 percent or 6,739 out of 8,766 were approved.   Figure 4. Number of received and approved annotated CRDs: 2020-2021   Omitted statement/information on the annotation records the most common problems/errors encountered during the processing of annotation in CALABARZON Some of the received civil registry documents were not processed due to problems/errors encountered during the processing. Such problems resulted to feedback to LCROs. In 2021 the most common errors encountered during the processing of annotation is Omitted information/statement in the annotation. This is followed by lack of requirements, documents not certified photocopy, wrong format of affidavit of supplemental report, and others (wrong supporting documents, error in the certificate of finality, etc.), respectively.     March recorded having the highest number of problem documents returned to LCROs In 2021, 852 CRDs with feedback were returned to local civil registry office for re-work or action. Feedback was highest in March,135 and lowest in May, 30. On the average, about 71 feedbacks to LCROs were prepared monthly. This is equivalent to about 3 civil registry documents per day.   Figure 5. Number of feedback by month   Quezon province recorded having the most LCROs with problem documents resulted to feedback Quezon as the province with highest number of submitted CRDs for annotation in 2021, also shared the greatest number of LCROs with submitted problem documents at 237. This was closely followed by the province of Laguna at 230, while Cavite, Batangas and Rizal shared 188, 176 and 19, respectively.
