Regular operations will resume on 30 April 2021. The general public are hereby advised of the following: Only clients who will claim their documents applied before the closure will be entertained by the Outlet. Please bring the official receipt (OR) issued during the application and valid ID. For representative, please bring the authorization letter issued by the document owner, valid ID of the document owner and representative. Also, do not forget to bring ballpen. During the 2-week closure, clients have the options to apply their civil registry documents (CRDs) at the Local Civil Registry Offices (LCROs), SM Batch Request Entry System (BREQS) Partners (SM City Batangas, SM City Lipa, SM Hypermart Rosario and Waltermart - Tanauan City) and thru online via www.psaserbilis.com.ph and www.psahelpline.ph . For follow-up of status of requests for loading of annotated civil registry documents thru the Decentralized Copy Annotation Process (DeCAP), please call (043)312-4815, 0905-075-3764 (Text Only) or you may e-mail psarsso4a@gmail.com Processing of Electronic Endorsement (“Piecemeal” or advance copy, clear copy of civil registry document) is also affected by the closure.
