The Philippine Statistics Authority (PSA) Quezon Provincial Statistical Office has been actively conducting house-to-house registration for the National ID System in the province of Quezon to assist elderly individuals and persons with disabilities (PWDs) in obtaining their National ID.
Special registration services are conducted at the residences of those who are unable to visit registration centers or mobile registration, such as bedridden elderly and disabled individuals.
This approach ensures that all citizens have the opportunity to register with the National ID System, regardless of their physical limitations. This effort not only simplifies the process but also highlights the importance of the National ID System in providing formal identification, which can facilitate access to government services and social protection programs.
Primary documents include Certificate of Live Birth, Philippine Passport, Unified Multi-purpose Identification (UMID) Card, or Driver’s License issued by the Land Transportation Office (LTO). Clients may also present (1) government-issued identification document bearing full name, birth date, birth place, front-facing photograph, signature or thumb mark. In addition, employee ID, School ID, barangay clearance/ID and City/Municipal ID are also accepted.
To request this service, coordinate with the PSA Quezon Statistical Office with telephone numbers (042) 373-7059 or (042) 717-3629 located at the 3rd floor Pacific Mall, Lucena City, Quezon or email request to quezon@psa.gov.ph.
