  • Sa mga naatasang maging representative para kumuha ng dokumento, ang mga sumusunod ang mga kailangang isumite sa PSA (For authorized representative, the following are the basic requirements which need to be submitted to the PSA):
  1. Original at Photocopy ng valid ID ng may-ari ng dokumento (Original and Photocopy of the valid ID of the document owner)
  2. Authorization letter o Special Power of Attorney (SPA) mula sa may-ari ng dokumento. Ang authorization letter ay dapat nakasulat sa isang malinis na papel, nagsasaad na ito ay gagamitin sa pagkuha ng dokumento sa PSA, uri ng dokumentong kukuhanin, dami o bilang ng kopyang kukuhanin, impormasyon ng may-ari ng dokumento, kompletong pangalan ng representative at dapat ay may fresh signature ng may-ari ng dokumento na kaparehas ng pirma na nasa valid ID. (Authorization letter or duly notarized Special Power of Attorney (SPA) issued by the document owner. The authorization letter should be written in a clean sheet of paper and dated; indicates type of document, number of copies and complete information of the document owner, complete name of the representative, fresh signature of the document owner that matches to his/her valid ID.)
Paalala:
Para sa Authorization letter/SPA na galing sa ibang bansa, ang may-ari ng dokumento ay dapat makapagbigay ng scanned/photocopy ng kanyang PASAPORTE bilang valid ID.
(Reminder:
For Authorization letter/Special Power of Attorney (SPA) received from abroad, the document owner should provide a scanned/photocopy of the PASSPORT as valid ID.)

  • Para sa mga Senior Citizen, Physically Challenged Person/PWD, Pregnant Woman, maari nilang gamitin ang Special Lane kung ang kanilang kukuhaning dokumento ay para sa kanilang sarili, sa kanilang legal na asawa, magulang at mga anak. (For Senior Citizen, Physically Challenged Person/PWD, Pregnant Woman, they can use the Special Lane if the documents they are requesting are their own document, document of their legal spouse, parents, child/children.
