What are the Requirements:
  1. Accomplished Application Form.
  2. Valid ID is required for both the owner and requester of document.
- Authorization letter and ID of the document owner together with the ID of the requester are required if the requester is not of the following:
  • document owner;
  • parent;
  • spouse;
  • direct descendant;
  • legal guardian/institution in-charge, if minor.

Kumuha ng numero at tamang Application Form (AF) sa Security Guard. Isulat sa AF ang mga hinihinging impormasyon. Ang requester ay kailangang pumirma sa espasyo na matatagpuan sa likod ng AF.
Magbigay ng application form (AF) sa client. 30 segundo depende sa requester Security Guard

Hitayin tawagin ang numero para sa Screening/Payment ng aplikasyon. Ang pagtawag ng numero ay by batch of 50 sa Regular Lane at by batch of 10 sa Special Lane. Siguraduhing kasama sa batch na tinawag ang numerong nakuha bago pumila sa screen/payment windows.

Kapag and requester ay hindi kanya ang dokumento na kukuhanin, ihanda ang authorization letter at valid ID ng may-ari ng dokumento kasama ang valid ID ng requester. Ang authorization letter o Special Power of Attorney ay derektang nagsasaad na ito ay gagamitin sa pagkuha ng dokumento sa PSA. Ang isang ID ay maituturing "Valid ID" kapag ito ay issued ng isang ahensya ng gobyerno tulad ng Passport, GSIS/SSS UMID, OWWA ID, Senior Citizen's ID, Voter's ID, Driver's license, PRC license/s, PhilHealth ID, at PAGIBIG ID.

Suriin mabuti ang AF kung kompleto, tama, naaayon at nababasa ang impormasyong isinulat ng aplikante. Hingiin ang valid ID ng requester. Kung hindi kanya ang dokumentong kukunin, hingiin ang authorization letter at valid ID ng may ari ng dokumento. Kung kompleto at walang problema sa mga requirements, hingiin sa kliyente and karampatang bayad para aplikasyon. Mag isyu ng resibo. Tatakan ng petsa ang resibo kung sakaling ang dokumentong kinukuha ay hindi mare-release sa araw ng pag apply.

Sa release ng dokumento, pumunta sa window kung ssan tinawag ang pangalan ng may-ari ng dokumento na kinukuha. Ihanda ang resibo, valid ID at authorization letter na hihingiin ng releaser.

Para sa release ng CENOMAR, BIRTH Certificate, Marriage Certificate at Death Certificate na iniapply ng ibang araw, pumunta sa Window 11, Ibigay ang resibo sa Releaser para mahanap ang dokumentong kiniclaim. Kailangang mag presenta ng valid ID ang may-ari ng dokumentong kiniclaim. Para sa naatasan na i-claim ang dokumento ng ibang tao, kailangan na makapagpresenta ng authorization letter, valid ID ng may-ari ng dokumento at valid ID ng naatasang kumuha ng dokumento.
Hanapin ang dokumentong kinukuha. Hingian ng valid ID at authorization letter bago i-release ang dokumento. 5 minuto

5 Para sa follow up ng dokumento na dumaan sa Electronic Edorsement at Annotation, pumunta sa Window 14 upang malaman ang estado ng inyong dokumento. Kung sakaling maari na kayong mag-apply ng kopya ng inyong dokumento, sundin and Step 1,2 at 3.

I-check ang status ng dokumento na pina-follow-up. Sabihin sa kliyente ang dapat na gawin matapos alamin ang estado ng dokumento.

6 Pumunta sa Public Assistance and Complaint Desk (PACD) kung mayroong katanungan sa kinuhang dokumento o may iba pang impormasyon na nais malaman. Sagutin at aksyonan ng tama ang mga katanungan at pangangailangan ng kliyente. depende sa klase ng assistance o complaint Care Officer/ Outlet Supervisor/ Outlet Manager
