Kapag and requester ay hindi kanya ang dokumento na kukuhanin, ihanda ang authorization letter at valid ID ng may-ari ng dokumento kasama ang valid ID ng requester. Ang authorization letter o Special Power of Attorney ay derektang nagsasaad na ito ay gagamitin sa pagkuha ng dokumento sa PSA. Ang isang ID ay maituturing "Valid ID" kapag ito ay issued ng isang ahensya ng gobyerno tulad ng Passport, GSIS/SSS UMID, OWWA ID, Senior Citizen's ID, Voter's ID, Driver's license, PRC license/s, PhilHealth ID, at PAGIBIG ID. |
Suriin mabuti ang AF kung kompleto, tama, naaayon at nababasa ang impormasyong isinulat ng aplikante. Hingiin ang valid ID ng requester. Kung hindi kanya ang dokumentong kukunin, hingiin ang authorization letter at valid ID ng may ari ng dokumento. Kung kompleto at walang problema sa mga requirements, hingiin sa kliyente and karampatang bayad para aplikasyon. Mag isyu ng resibo. Tatakan ng petsa ang resibo kung sakaling ang dokumentong kinukuha ay hindi mare-release sa araw ng pag apply. |

Para sa release ng CENOMAR, BIRTH Certificate, Marriage Certificate at Death Certificate na iniapply ng ibang araw, pumunta sa Window 11, Ibigay ang resibo sa Releaser para mahanap ang dokumentong kiniclaim. Kailangang mag presenta ng valid ID ang may-ari ng dokumentong kiniclaim. Para sa naatasan na i-claim ang dokumento ng ibang tao, kailangan na makapagpresenta ng authorization letter, valid ID ng may-ari ng dokumento at valid ID ng naatasang kumuha ng dokumento.
